Honoree Nomination Form

This form should be used to submit a person or group to be honored at a local event during the Lone Star Circle of Life Bike Tour.

Honorees should have a connection with one of the three types of donation the tour supports – blood, marrow/stem cell, or organ/tissue.
The honorees may be donors, recipients, someone searching for a donor, or a person or group who has contributed to the field of donation.
A person or group is welcome to submit their own name.
If you are submitting the name of another person or group, please check with that person, or their family, to get permission to submit their name. Make sure to include contact information so the tour coordinator can obtain the information needed for the website.
Anyone or group being honored will be asked to sign a publicity consent form, submit a brief story about them and supply a photograph to the tour to use on the honoree page of this website.

Once this form is submitted, the tour administrator will contact you for additional information.
